Advantage is a provider for the following insurances. We recommend you contact your insurance carrier's customer service to determine your benefit for out–patient Physical Therapy services. Please bring your insurance card and referral information required by your insurance to your first visit. Contact our office for further assistance.
Worker's Compensation and Motor Vehicle Accident claims require additional information in order to obtain approval for physical therapy services.

Co–payments are expected at the time of visit. We accept MasterCard, VISA, American Express, and Discover credit cards and debit cards (HSA, HRA) that bear a VISA or MasterCard logo.
Motor Vehicle Accident
You will need to provide the following information for all motor vehicle accident claims.
- Your personal auto insurance carrier name (not the other party's information)
- Claim number
- Date of injury
- Insurance adjuster's name
- Insurance adjuster's phone number
- Your personal health insurance information (needed to coordinate benefits between both insurances)
Worker's Compensation
You will need to provide the following information for all workers compensation claims.
- Employer's Name
- Insurance Company Name
- Insurance adjuster's name
- Insurance adjuster's phone number
